Evaluation Portfolio Outline
Overview & Layout: This resource is meant to offer teachers a guideline to follow in constructing their Evaluation portfolio. This outline includes all Focus Areas established by the Leadership Team. Divide your portfolio into THREE segments including the following: Focus Area - Planning/Preparation, Focus Area - Instruction, Focus Area - Professional Responsibilities. Each SEGMENT should include a Title Page, Table of Contents, Context Piece, and Brief Reflection.

Focus Area: Professional Responsibilities
1. I-Plan (Evidence of life-long learning) - If you have and are obligated to follow the I-Plan, include this here and cite your progress.
2. Professional Development (Integration of Professional Development) - Any professional development seminars, conferences, like Quest would work out well here. Show evidence that you used this in your teaching!
3. College Classes & Readings (Evidence of Life-long learning) - Transcripts, articles or books you've read regarding your teaching (A book abstract would suffice here)
4. Parent Communication (Effective communication with families) - Any of the following would be helpful as evidence:
A. Parent/Family Communication Log
B. Team or Class Newsletter
C. Class Website or Webpage (print it out)
D. Class Overview or Syllabus
E. Field Trip Forms and Pictures
F. Parent Progress Forms (Example: Anytime Progress Report)

Each section should include a brief context piece and reflection. The reflection should be on the body of evidence included within the Focus Area.
